Build a Higher intent instant form with qualifying questions and consent
Create a Higher intent instant form with three qualifying questions, your privacy policy, a contact consent disclaimer and a completion screen that sets expectations.
Video walkthrough
- More volume vs Higher intent, and why Realtors want the review screen
- Three questions that qualify without touching protected characteristics
- Privacy policy and the consent disclaimer for calls and texts
- A completion screen that tells the lead exactly what happens next
- Why the form locks once finished, and how to change it later
What you're building
An instant form named SELLER_HI_v1 (or BUYER_HI_v1) with form type Higher intent, an intro, three qualifying custom questions plus name, phone and email, your privacy policy URL, a custom disclaimer with a consent checkbox, and an ending screen that names who will call and when.
Why it matters
Meta describes Higher intent forms as adding inline context under contact fields saying the business may follow up, and an extra review screen where people confirm their information. Meta suggests it when each lead needs follow-up or further qualification, which is every Realtor lead. Fewer accidental submissions mean fewer wrong numbers and less time wasted by whoever calls.
The questions do the first round of qualifying for you, and the consent disclaimer documents that the person agreed to be called and texted about this specific request.
What you need
- A privacy policy page on your website (a web page, not a PDF). Meta's help suggests it cover what you collect, how you use and share it, how people can review or delete it, and how to contact you
- Your qualification rule from chapter 1
- Your speed target and business hours from chapter 1
- Consent wording reviewed by your broker or attorney, especially if you will text leads
Time and cost
- Time
- 40 minutes
- Cost
- $0
Attorney review of consent language, if you choose it, is a separate cost.
Step by step
Open the form builder from the ad
In your first draft ad, find the Instant form section and click Create form. Build the form in Ads Manager, not Meta Business Suite: Meta notes that custom questions, conditional answers and phone verification are only available when the form is created in Ads Manager. Name it
SELLER_HI_v1. Keep form names under 64 characters, because Follow Up Boss does not create a tag for longer form names.Check: The form builder is open and the form name is set.
Set Form type to Higher intent
In Form type, change the default More volume to Higher intent. Meta states a Higher intent form only delivers to Facebook Feed and Instagram feed on mobile devices, not desktop.
Check: Form type reads Higher intent.
Write the intro
Add a headline and a short description that say exactly what the person gets and who provides it. Use the intro from the form template below.
Check: A stranger reading only the intro could say what they will receive and from whom.
Add three qualifying questions plus contact fields
Add custom questions for ownership (or buying timeline), address (or areas), and timeline, then a preferred contact method. Use multiple choice where you can; Meta notes you must enter each possible answer. Then add prefill questions for Full name, Phone number and Email.
The buyer question "Have you spoken with a lender yet?" asks about a step, not about finances. Do not add follow-ups about income, down payment or credit.
Check: The form has 3 or 4 custom questions and name, phone and email prefill, and none of the questions touch the prohibited list.
Add your privacy policy and the consent disclaimer
- In Privacy policy, enter your privacy page URL and link text.
- Under Custom notices, click + Add custom disclaimer. Add a title and text naming who will contact the person, about what, and by which channels (call, text, email), with how to opt out.
- Click + Add new consent to add a checkbox. Leave it required.
Meta notes that once a disclaimer is added, people must agree to submit the form, and that you cannot add a disclaimer to a form already running in an ad; you would duplicate the form instead.
Check: The preview shows your privacy link and a required consent checkbox with your name and brokerage in the text.
Write the ending screen
In the Ending section, write who will call, from which number, and within what time. Add a button to your website. A lead who expects your call from a known number is more likely to answer it.
Check: The ending names a person, a phone number and a time window.
Finish the form and confirm the ad is still a draft
Preview every screen, including the Higher intent review screen. When you finish the form, Meta locks it: Meta's help says a form can be edited only while it is a draft, and changes later require duplicating it. Finishing the form does not publish your ad.
Attach the same form to all three ads. Close the editor and confirm the campaign still shows draft or off and $0.00 spent.
Check: All three ads reference SELLER_HI_v1, and the campaign shows $0.00 spent.
What success looks like
- Higher intent form with intro, 3 to 4 qualifying questions and contact prefill
- Privacy policy URL and required consent checkbox
- Ending screen naming who calls and when
- Attached to all three draft ads
Copyable templates
Consent text is a starting point for your broker or attorney to review, not legal advice.

Q2 (short answer): Which cities or areas are you considering?
Q3 (multiple choice): Have you spoken with a lender yet? Yes / Not yet / Paying cashHow to test it
In the form preview, step through as a lead: tap each answer, reach the review screen, confirm the consent checkbox is required, and read the ending. In chapter 9 you will submit a real test lead with Meta's Lead Ads Testing Tool and watch it arrive in your CRM.
Troubleshooting
I need to change a question after finishing the form.
Duplicate the form, edit the copy, name it _v2, and attach it to the ads. Update your CRM field mapping for the new form in chapter 9.
The privacy policy link is rejected.
Meta does not accept links directly to a PDF, image or download. Publish the policy as a web page and link to that.
I do not see Higher intent.
Confirm you are building the form in Ads Manager under a Leads campaign with Instant forms as the conversion location. Business Suite only builds simple forms.
Advanced technical details
Optional. You can follow every step above without these.
Technical detailsOptional quality features
Ads Manager forms also support conditional answers, conditional logic (different next question by answer), and phone number verification by one-time passcode before submission. Phone verification can lower wrong-number leads at some cost to volume; test it in a second round, not the first, so you change one variable at a time.
Meta's help says you can save up to 100 instant forms; when you create one beyond that, only the most recent 100 are kept. Name forms by goal, type and version so the active one is obvious.
